Qlarant publishes the instrument its reviewers fill in. It is 99 pages and 51 standards, it is free, and almost nobody opens it. This is all of it in plain English — including which failures are automatic Alerts, and honestly which of them Care Wizard helps with.

General Administrative

Nine standards about the agency rather than any one staff member. The first one is scored before anything else in the review.

1. Your staff roster lives in the Clearinghouse and is correct

An APD Employee/Contractor Roster inside the AHCA Background Screening Clearinghouse, showing names, licence number, position type, provisional and permanent hire dates, retained prints expiration and end date. An Excel export of it is not accepted, and an expired retained prints date fails on its own.

We track the hire and screening dates the roster has to agree with. The roster itself lives in the Clearinghouse and only you can maintain it there.

2. Agency vehicles are insured

Insurance covering the entire period of review for every agency-owned vehicle used to transport people — which can mean three six-month policies. Current on the day but lapsed mid-period still scores Not Met.

3. Agency vehicles are registered

Qualifications and Training

Twenty standards, applied to each staff member. This is where most Alerts live, and almost all of it is dates.

3. Zero Tolerance

4. Direct Care Core Competencies

Once, within 90 days of providing services. TRAIN Florida only since 17 September 2024. Older two-part versions and the separate Person-Centered Planning and Individual Choices courses are scored instead only where the 2016 curriculum was never taken.

5. Requirements for all Waiver Providers

Once, before starting work, in TRAIN Florida since 1 November 2022. Provider Advisory #2022-027 requires it of all providers and provider staff who render services through the iBudget Waiver — which includes residential habilitation, so group home staff are in scope.

Tracked under the name the course actually carries in TRAIN Florida: APD Requirements for All Waiver Providers. Search the Course Catalog for that exact phrase.

6. HIPAA

Within 30 days of providing services, then updated at least annually — within 365 days, not merely in the same calendar year.

7. HIV/AIDS and Infection Control

Within 90 days of providing services, from an APD approved source. Not required for Behavior Analysis providers. If the card carries an expiry, renewal must precede it.

8. CPR

Within 90 days, and it must be a classroom certification — the tool states this cannot be completed online. Not required for Behavior Analysis.

9. First Aid

10. Basic Medication Administration training

Completed before administering or supervising self-administration, taught by an APD-authorised RN, APRN or LPN, documented on APD Form 65G-7.003 B. Required again after a lapse in primary route validation.

11. Medication administration validation

The single most detailed standard in the tool, with 27 Not Met reasons. The primary route must be circled and dated; revalidation must fall in the 60 days immediately before the expiration date — not earlier, not later — and primary route validation must happen within 180 days of the training.

The 60-day window is the one that grounds med techs.

12. Medication administration annual update

Completed before re-validation and within the 60 days preceding it, documented on APD Form 65G-7.004 A.

13-15. Prescribed enteral formula: training, validation, annual update

Care Wizard tracks this

A separate track on top of basic medication administration, with its own training, its own validation certificate and its own annual update inside the same 60-day window.

16. Temporary validation, where one was used

Never longer than 30 days, never for enteral formula, and the permanent validation must follow within 30 days of the temporary one being signed.

A short-lived document tied to one person and one medication, which is nearer to a care record than a staff credential.

17. Reactive strategies / behavioral emergency curriculum

An APD-approved crisis curriculum within 30 days of serving anyone who may need behavioral emergency procedures. The certificate is valid one year and renewing means the full curriculum again, not a refresher.

18. Drivers hold a valid licence

A licence valid across the entire period of review, which can mean two licences. Not current at the review is an Alert; lapsed mid-period is Not Met without one.

19-20. Personal vehicles used for transport are insured and registered

Cover across the entire period of review — twelve full months, which can mean two or three insurance cards — for every personal vehicle used to transport people.

Service Specific Employee

The remaining standards run service by service, and they repeat the same three things each time: minimum education and experience, any pre-service course, and an annual in-service hour count. Listing thirty-one near-identical rows would be worse for you than the table below, so here is what actually differs.

The in-service year is not the year most people assume.

Every one of these standards says the 12-month window is set by the staff member's date of hire, unless you can demonstrate another well-documented revolving 12-month period — and states outright that it cannot be based on the annual Provider Review date. Re-taking a basic APD course does not count toward the hours unless your Plan of Remediation says so, and a certificate without the credit hours printed on it is a listed Not Met reason.

Why the last column is honest

Care Wizard tracks dates — the trainings, screenings, validations and renewals a reviewer asks for against your roster. It does not hold incident reports, medication error reports, or anything at all about the people you serve, and it does not track your vehicle policies. 9 standards above say not tracked, and they will keep saying so.
